Requesting a Transcript

  • This form is for alumni to request an official or unofficial transcript.

    To request a transcript from Gates Chili Central School District, please complete the form below. The transcript will be sent to the address you specify. Questions about transcripts may be directed to the High School Career Center at 585.247.5050 ext. 21522.

    The information that you will need to complete the form includes:

    • Name 1 (at time of attendance at Gates Chili)
    • Name 2 (used on college or employment application)
    • Year of Graduation (or withdrawal) from Gates Chili
    • Type of record you are requesting
      • Official Transcript: Can only be sent to a school or work place and includes all grades and testing scores.
      • Unofficial Transcript: Can be sent to the former student and does not include the testing scores 
    • Complete address to send transcript
    • Your contact information: telephone number, email address
    • A notarized signature 

     

    Important note: Requests will not be accepted without notarization.
