Before you begin the registration form, please gather the following documents to submit. You must attach a scanned copy of these documents when you submit your registration forms via email.

• Proof of a student’s address (mortgage statement, rental or lease agreement, tax receipt)
• Documentation of age (birth certificate, a record of baptism, passport)
• Custody, guardianship and/or adoption documents, if applicable
• Record of immunizations and current physical. These may be transferred from your student's current school. This is required for part two of the registration process but may be submitted during part one. Learn more about NYS Immunization requirements at this link.

Download one of the following fillable registration forms and save it to your computer or mobile device. Complete all pages of the form, including your email address and your phone number. Use the electronic signature to sign and date the forms before saving:

• Student Registration Form Part I - English
• Student Registration Form Part I - Arabic
• Student Registration Form Part I - Spanish
• Student Registration Form Part I - Ukrainian
• Student Registration Form Part I - Vietnamese

Once the form is completed, signed and saved, email it to the district registrar at registrar@gateschili.org and attach the following items to your email:

• completed and signed student registration packet (part one)
• proof of student’s address
• documentation of age
• custody documents, if applicable

Once you have emailed your registration form and supporting documents, if everything is complete, you will receive a confirmation email from the registrar with the next steps for enrolling your student.

Alternatively, you may print the registration form and complete by hand, then scan and email to the above email address with your supporting documents.
