Ms. D'Angelo, the School Nurse along with Ms. Mosher the Health Aide are here to aid parents, guardians and the classroom teacher in maintaining each child's total well-being- physical, social and emotional.

Immunizations
In accordance with the New York State Public Health Regulations, no student will be allowed to attend school without a complete and up-to-date immunization record on file with the District.

For families who are new to the Gates Chili School District, this means that you may not register your child for school without providing a current immunization record from a physician. For families with children currently attending school in the Gates Chili School District, please check with your pediatrician or school nurse to be sure that your child's immunization record is complete and up-to-date.

Any student with an incomplete immunization record will be required to receive further immunizations before attendance at school is permitted. Documentation of your child's immunizations must be on file with the district before the first day of classes or your child will be excluded from school until this is received.

Public Health Law, Section 2164, requires all students to be immunized against the following diseases before entry to school:
- Diphtheria (DPT)
- Polio (oral)
- Measles, Mumps and Rubella (MMR)
- Hepatitis B
- Varicella (Chicken Pox)

Medication
Medication is administered to a student only when the family physician considers it absolutely necessary to prevent serious and perhaps fatal consequences (severe allergy to bee stings, for example). In such cases, the Nurse must have a written order (this includes over-the-counter medicine as well as prescriptions) from the doctor plus a written request from the parents. The medicine is kept in a safe place in the Health Office.
The following conditions must be in place:
- A written or faxed order from the physician is provided-even for over-the-counter medications such as Tylenol or cough drops.
- Written permission from the parent or guardian is provided.
- The medication must be in the pharmacy filled prescription bottle or the original store packaging.
- The medication must be brought into school by an adult.
- A new order is required each school year.

Physical Education Excuses/Restrictions
A note from a physician is required in order to excuse your child from class or restrict their activity for more than 2 consecutive phys ed classes. An excuse for PE also restricts playground activity for the length of the excuse.

Physical Exams

New York State Education Law requires students to have a physical exam on record in the health office during elementary school. Physical exams are required for all students, who are:
- Entering Kindergarten
- Entering 1st, 3rd and 5th grades
- New to the Gates Chili School District

Copies of physical exam records must be submitted to the Health Office within 30 school days after school begins. It is recommended that these examinations be done by the family physician since they are more familiar with the health needs of the child. However, with your permission, our District School Nurse Practitioner will complete a screening physical free of cost. If we have not received your privately completed physical within 60 days, we will schedule one with our Nurse Practitioner.

Screenings

Annual screenings are completed in the early part of the school year. If a child fails any of the screenings, parents/guardians are notified and follow-up with a personal physician is recommended. At any time throughout the school year, if a vision or hearing problem is suspected, a screen may be requested by a parent or a teacher.
The screening schedule below will be followed.
- Kindergarten and new students to the District will be screened for far, near, color vision and hearing.
- 1st and 3rd graders will be screened for far and near vision and hearing.
- 5th graders will be screened for far and near vision, hearing, and scoliosis.

Sickness Guidelines
Please do not send a child to school with signs and symptoms of illness. Some specific situations are:
- A fever of 100.3 degrees or higher. (Please do not give Tylenol or ibuprofen and then send your child to school. The cause of the fever does not go away, and your child could still be contagious to others.)
- Any rash not diagnosed by a physician.
- Head lice, scabies, pinworm and ringworm, until treatment has begun.
- Diarrhea or vomiting that morning prior to school.
- Strep throat until the result of the throat culture is known and if positive, your child has been on antibiotics for 24 hours.
- Chickenpox, until the scabs heal over (about a week).
If a child becomes ill or is injured in school, the nurse will notify the parent/guardian. Emergency first aid is the only treatment that may be administered in school and is administered only for school-connected injuries. Responsibility for further treatment rests with the parent/guardian. If an ambulance must be called, the parent/guardian will be contacted first, if possible.
