Welcome to Gates Chili Middle School Health Office

 

The Health Office is staffed by Mrs. Tyna Fowler RN, and Health Aides Karen Jefferson and Christine Gear. The following information is intended to explain school health services and policies.

 

PHYSICAL EXAMINATIONS

 

New laws governing confidentiality have impacted the school's ability to gather necessary medical information directly from physician's offices. The easiest way to transfer information is through you. Your effort to ensure that the school receives required documentation will be greatly appreciated.

 

Education Law, Section 903, requires a physical examination of all 7th grade students and new entrants. Students are encouraged to have these exams done by their family physician; they are more familiar with the health needs of your child. However, our school physician will complete a physical for those in need.

 

SCOLIOSIS SCREENING

 

As required by New York State Law, all students 6th through 8th grade need to be screened for scoliosis. Scoliosis is a curvature of the spine which can occur in boys and girls as they are growing. It will be done once a year to those students of whom we do not have proof of it being done by their pediatrician. Parent or guardian will be notified if there child does not pass the screening; if positive screening, a further evaluation by a pediatrician is needed.

 

IMMUNIZATIONS

 

Public Health Law, Section 2164, requires all pupils to be immunized against the following diseases:

-DPT (Diptheria, Pertussis and Tetanus) 3 doses

 

-DT or TD needs to be updated every 10 years

 

-Polio (3 doses)

 

-Mumps (1dose)

 

-Rubella (1dose)

 

-Measles (2doses)

 

-Hepatitis B (3 doses)

 

-Varicella or Proof of Chicken Pox (children born on or after 1/1/94)

 

-Tdap(Tetanus, Diptheria, Acellular Pertussis) for students entering 6th grade after 9/1/2007 and 11 years of age or older 

 

MEDICATIONS

 

Students sometimes need medication during the school day; Medication dispensed by school personnel only under these conditions:

 

-A written order from your physician.

 

-A written request form signed by the parent or legal guardian.

 

-Medication must be in the prescription bottle and brought in to the Health Office by an adult. This includes any over the counter medicines.

 

Please do not send medications into school with your child. The medicine will be confiscated by staff if it is brought to school by a student. Medication will not be returned to students under any circumstance. Parents will need to pick up the medicine from the health office.

 

Students are responsible to come at the appropiate time for medications.

 

SPORTS CLEARANCE

 

Any student who is interested in participating in a 7th or 8th grade sport is required to:

 

-Fill out an Interscholastic Athletic Program and Medical Information form prior to each sport season.

 

-Student must have a current physical on record.

 

After review of the Health Record and information sheet, students will be cleared to participate in their chosen sport. Any student needing to see the school physician, must make an appointment with the Health Office to be seen by the school physician at school.
